Arriving with clean eyes (no strip lash glue, mascara, etc.) will make the service move a lot quicker!

If you are more than 15 minutes late to service,
the appointment will be automatically canceled.
- Due to our policy, we ask that you do not bring any extra guests.

Not giving your lash technician a minimum of 8 hours' notice makes it harder to fill in for the appointment. For any reason you need to reschedule or cancel an appointment, you must communicate that 8 hours prior, and your deposit will be put towards your next appointment. Providing less than eight hours' notice will forfeit your deposit. Not providing any notice whatsoever will also forfeit your deposit. Let us know if you have any questions!

LASH EXTENSIONS COMPLEMENT BUT DON'T DAMAGE YOUR NATURAL LASHES; BE SURE NOT TO PULL/PICK/TWIST, AS THIS WILL DAMAGE THEM.

CLEANSE LASHES DAILY WITH AN APPROVED CLEANSER

TO MAINTAIN A FULL LOOK, FILLS AT THE 2-3 WEEK MARK ARE RECOMMENDED.
With my signature below, I attest that I have read the previous information and details from above. I assume all in full responsibilities for any risks or injuries, losses, side effect damages, that may occur as part of the procedure. I will not hold my eyelash technician (Name printed below) responsible for any conditions present at the time of treatment but not disclosed that may affect the treatment. I do not hold my technician responsible for any damages or loss that may occur as a result of not following proper aftercare instructions.

Disclaimer & Consent
Allergic reactions to eyelash extensions are caused by an allergy to the cyanoacrylate base bonding agent) that is basically in every eyelash extensions adhesive. If you are developing an allergy to the adhesive you may experience swelling, itchy eyes, and/or redness. The situations are rare, although we do ask that you do not book the service if you have previously experienced allergies to lash adhesive or have really sensitive eyes.
